ProActive Orthopedic and Sports Physical Therapy is an outpatient rehabilitation clinic which emphasizes individualized, hands-on care.

The goal of ProActive Orthopedic and Sports Physical Therapy is to provide residents of Oregon and SW Washington with a locally owned and managed physical therapy facility. We currently have locations in Gresham, Oregon City, Canby, Vancouver, and Myrtle Creek.

  • Patients treated by a licensed therapist or licensed assistant each office visit.
  • Extended office hours to accommodate the working patient
  • Monday through Friday 7:30am to 6:30pm.
  • Immediate appointment.
  
  • Patients seen within 24 hours of referral or on a walk-in basis when requested.
  • Personalized Evaluations and Plans of Care.
  • No computerized templates.
  • Initial evaluation and progress reports promptly provided in addition to ongoing verbal communication.
  • Most Insurances accepted.
